40 hours/full-time Monday through Saturday. Hours will vary per availability of individuals on caseload.
Position Description:
Job Purpose:
To provide outreach, engagement and coordination of services to individuals who are incarcerated and part of the Lancaster County Prison’s Medically Assisted Treatment program.
Essential Duties:
- Establish connections and provide supportive peer services to inmates who are enrolled in the Prison’s Medically Assisted Treatment Program (Suboxone, Subutex, Sublocade and Methadone).
- Establish strong working relationships with the Prison’s Treatment Services Staff (Inmate Services, Reentry, Drug and Alcohol and Work Release staff) to support the facilitation of inmate motivation, education, awareness and connection to services post release.
- Collaborate with appointed SCA Case Managers in fulfilling the recommendations from ASAM Level of Care Assessment.
- Assists Prison Reentry staff in the preparation and execution of the inmate’s plan for release and home plan, including connection to supportive and treatment services the inmate will utilize in the community. These services might include recovery support programs, drug and alcohol treatment services, mental health support services, employment training, housing, and primary health care services.
- Completes a “warm handoff” process with outside support or treatment agencies/individuals as the inmate transitions into the community and performs follow up to ensure continuity of referral services.
- Coordinate with Adult Probation and Parole as necessary to ensure continuity of outside services and support.
- Maintains and submits Data as required including case notes, participant demographics, referral information etc.
- Develop and review Recovery Plans.
- Maintains an understanding of State and Federal Confidentiality Laws as they relate to Drug and Alcohol Treatment programs (42CFR, Part 2, Act 33 of PA code)
- Completes and maintains security training for the Lancaster County Prison and can successfully complete a background screening.
Job Requirements:
Education, Experience, & Qualifications:
- High School Diploma/ GED
- Forensic Peer Support, Certified Recovery Specialist, Certified Peer Support certification is strongly recommended.
- Desirable that the applicant understands peer support ethical and practice guidelines of being open minded, recovery oriented, hopeful, respectful, empathetic agent of change, strengths focused, person centered.
- Must be able to supply professional letters of support/reference from a minimum of 3 sources.
- Must have experience in the behavioral health field working with individuals in substance use disorder recovery who are also utilizing medically assisted treatment as recovery support.
- Must understand Medication Assisted Recovery, including suboxone, Subutex, Sublocaid and methadone treatment and must be able to provide supportive services in an unbiased manner that respects individual choice.
- Applicant should be able to function effectively in an environment that is tightly controlled and manage high stress situations in a calm and professional manner.
- Must have a strong personal support system in place.
- Desirable that the applicant has a strong optimistic and positive disposition.
- Bilingual candidates are strongly encouraged.
- Must have a valid PA Driver’s License
- Must be willing to obtain professional certification within 1 year of hiring (FPS, CRS, CPS)
- All candidates meeting the criteria will be considered however, we would prefer candidates with lived experience from a substance use disorder who also have involvement with the criminal justice system. Individuals who identify as being in recovery must also have 2 years of recovery and at least that long removed from any incarceration.
